Qualifying chronic conditions include diabetes, cardiovascular disorders and chronic heart failure. Not all members (even those with a qualifying condition) may qualify. Eligibility is based on meeting the Centers for Medicare and Medicaid Services' definition of "chronically ill enrollee" and all applicable plan coverage criteria.
